Under the supervision of the HARM:LESS Program Director, the Coordinator has knowledge of and commitment to practice harm reduction in all interactions with program participants and at all stages of program planning processes. Has experience in street-level outreach efforts for at risk populations. Significant knowledge of health-related issues faced by injection drug users, including awareness of injection drug user networks, norms, and culture. At least one year related experience working with individuals with a substance use and/or mental health disorder, unhoused, as well as HIV/AIDS.
Responsibilities will include:
- Provide syringe access and safe use supplies to people who use drugs (PWUD)
- Provide Rapid HIV/HCV testing for participants
- Provide linkage to care for HIV/HCV positive individuals

The Hope Not Handcuffs (HNH) Program Coordinator plays a key leadership role in supporting the HNH Program Director and overseeing daily operations of the Hope Not Handcuffs Call Center. This position ensures that individuals seeking help for substance use disorder receive timely, compassionate, and effective assistance. The Program Coordinator supervises call center staff, assists with data collection and reporting, and collaborates with community partners to ensure smooth coordination of services across FAN programs, including Healthy Families and police training for Deflection and Diversion programs.
Responsibilities will include:
- Provide day-to-day supervision and support for Hope Not Handcuffs Call Center staff to ensure quality service delivery, adherence to protocols, and positive participant outcomes.
- Work closely with the HNH Program Director to plan, implement, and evaluate program initiatives and operational improvements.
- Assist in managing cross-program collaboration, including supporting the Healthy Families Program, police trainings and other FAN initiatives as needed.
- Generate and analyze program data and reports for internal use and external stakeholders, ensuring accuracy and timeliness.
- Monitor Call Center analytics (call volume, call length, response times, outcomes, etc.) and develop recommendations to improve efficiency and participant engagement.
- Assist with staff training, scheduling, and ongoing professional development to promote a supportive and knowledgeable team environment.
- Help maintain and update the program’s master spreadsheet and data tracking systems.
- Communicate regularly with community partners, treatment providers, and law enforcement to strengthen referral networks and enhance collaboration.
- Support the Director in preparing presentations, training’s, and outreach efforts to expand awareness of Hope Not Handcuffs and related programs.
- Ensure program documentation, policies, and procedures remain compliant with organizational and grant requirements.

The Redirect addiction-related diversion program is a street-based, long-term, and unconventional approach to case management. Redirect case work is non-coercive, non-time delimited, street-based, and built on multiple pathways to recovery, including harm reduction, and steeped in Stages of Change theory and motivational interviewing practices. This position will manage a caseload of approximately 15-25 participants and will work in conjunction with local police departments and FAN’s Comeback Quick Response Team (QRT). Our team works to identify individuals impacted by substance use disorder (SUD) and provide a continuum of care to ensure participants get the treatment, support, and ongoing connection they need for their recovery journey.
Responsibilities will include:
- Meeting participants where they're at, both throughout the community and in their recovery process (in-home case management).
- Transporting participants.
- Promoting the development of a comprehensive service delivery system by identifying participant needs and gaps in services to meet those needs better.
- Collaborating with FAN’S Quick Response Team while carrying out duties of case manager.
- Providing assessments for participants within the program and meeting with participants shortly after identified (within 24 hours).
- Conducting initial assessments of the client’s needs, desires, strengths, and barriers.
- Together with the client, preparing the necessary action steps for the client and case manager to achieve established goals.
- Setting up regular follow-up meetings with the client, based on the client’s needs for intensity – but no less frequently than once per week.
- Assisting participant in accessing services, resources, and supportive benefits, as needed.
- Intensive case management, harm reduction strategies, identifying proper treatment, and following-up across 12 months with each participant.
- Following up, recommending exit, or reevaluating plan, as needed.
- Attending and participating in case review meetings with Case Management Supervisor at FAN.
- Tracking and developing appropriate data measures for program evaluation on electronic management system.
- Supporting and effectively communicating with team members, including employees and contractors.

- Bachelor’s degree in the human services or related field, with a proven track record of successfully working with diverse populations. Please note the FAN will consider alternative, comparable experience to a degree.

FAN is seeking to hire a Family Recovery Coach (FRC) to work in Bay City. This is an independent contractor position with a flexible schedule. Hours are on an as-needed basis and not guaranteed. The applicant must have lived experience as the family member of someone with substance use disorder. The FRC will be connected to and work with the families of loved ones suffering from substance use disorder, and, combining their experience with training and supervision, will guide families in responding to their loved one in a way that is healthy and supports family recovery.
