Nurse Practitioner/Physician Assistant—Street Medicine-Addiction Medicine (Part Time)

The Advance Practice Provider (APP) will support FAN’s expanding medical and recovery initiatives by providing direct clinical care and medical oversight for individuals impacted by substance use disorder (SUD) and related health conditions. Working in collaboration with the Medical Director and HARM:LESS Program Coordinator, the APP will address primary care needs that often intersect with addiction, such as wound care, hepatitis C (HCV), sexually transmitted infections (STIs), and other preventable or chronic conditions.

Key Responsibilities:

  • Provide patient-centered medical assessments, screenings, and follow-up care for conditions commonly associated with substance use, including HCV, HIV, STIs, and wound care.
  • Collaborate with the Medical Director and Director of Harm Reduction Services to develop and implement integrated care protocols for individuals served through FAN’s clinical and outreach programs.
  • Prescribe and manage medications within the scope of practice and in accordance with state and federal guidelines.
  • Participate in multidisciplinary case discussions and coordinate with peer recovery coaches, therapists, and community health partners to ensure continuity of care.
  • Assist with quality assurance, grant reporting, and outcome tracking related to FAN’s clinical programs.
  • Maintain thorough, timely, and compliant documentation in accordance with FAN and funding standards.
  • Provide education and consultation to FAN staff, partners, and community members on medical issues related to substance use, infection prevention, and wellness.
  • Managing and supervising staff medical staff including RN/LPN/MA: will designate proper CME, charting and coordinate schedules.

Qualifications, Skills & Experience:

  • Must hold a current and active Nurse Practitioner license issued by the State of MichiganPhysician OR a Physician Assistant (PA) license issued by the State of Michigan.
  • Addiction medicine experience preferred
  • Minimum of 2 years of experience in primary care, infectious disease, behavioral health, or substance use treatment.
  • Familiarity with trauma-informed care, harm reduction, and public health approaches.
  • Supervisory or mentorship experience with nursing or clinical staff preferred.

Quick Response Team Supervisor—Detroit QRT

Face Addiction Now (FAN) is seeking a dedicated and experienced Supervisor to lead the Detroit Quick Response Team (DQRT). This position is essential to the day-to-day functioning of the team and requires hands-on participation in field activities. The Supervisor will manage team schedules, coordinate operations, and ensure the effective delivery of services.

This role requires strong leadership, sound judgment, and experience managing teams in fast-paced, high-acuity environments. Lived experience related to substance use, recovery, homelessness, or justice involvement is highly valued and aligned with Face Addiction Now’s  mission, but management experience is the primary requirement for this role.

Key Responsibilities:

  • Oversee the daily operations of the Detroit Quick Response Team, ensuring that all activities align with FAN’s mission and organizational goals
  • Develop and manage team schedules in collaboration with team members to ensure adequate coverage and timely response to community needs
  • Actively participate in weekly outreach alongside the team, providing on-the-ground leadership, support, and guidance during interventions and community interactions
  • Work closely with FAN leadership to refine and enhance program strategies to meet community needs and organizational objectives

Qualifications, Skills & Experience:

  • Bachelor’s degree in human services, social work, psychology, public health, criminal justice, public administration, or a related field preferred, but not required. Equivalent professional experience will be considered
  • Minimum of 3 years supervisory or management experience, preferably within the substance use services, harm reduction, public health, EMS-adjacent work, or community-based social services
  • Demonstrated ability to lead, supervise, and support staff in complex, high-acuity, and community-based environments
  • Experience with EMS, first responder collaboration, and community outreach programs strongly preferred
  • Strong organizational and project management skills, including scheduling, planning, and operational oversight
  • Ability to analyze data and program outcomes and use that information to guide program improvements
  • Excellent verbal and written communication skills, with the ability to engage effectively with diverse populations and stakeholders
  • Strong conflict resolution and crisis management skills, including the ability to deescalate challenging situations
  • Experience working in diverse communities and understanding of the structural, cultural, and socio-economic factors that impact substance use and recovery

Outreach Specialist/Peer Recovery Coach—Detroit QRT 

The Detroit Quick Response Team (QRT) is responsible for responding to all non-fatal overdoses by providing immediate outreach at the doors of individuals who have recently experienced an overdose. In addition to direct outreach, the team also responds to hospital referrals and conducts proactive community outreach efforts.

The QRT model focuses on rapid, compassionate engagement following non-fatal overdoses, hospital referrals, and proactive community outreach, with the goal of reducing overdose-related mortality and increasing connections to care. As a Peer Recovery Coach, this role uses lived experience to support individuals in developing and sustaining their own recovery pathways.

This position requires comfort working in community settings, including entering homes, engaging individuals experiencing homelessness, and responding to high-acuity situations using trauma-informed, person-centered approaches.

Key Responsibilities:

  • Conduct outreach and engagement with individuals experiencing substance use disorders.
  • Provide education on harm reduction strategies, including safer use practices and overdose prevention.
  • Distribute harm reduction supplies such as naloxone, drug testing strips, sterile supplies, and hygiene items.
  • Conduct basic health assessments and screenings as needed.
  • Provide referrals and warm-handoffs to healthcare, recovery, and social services providers.
  • Work collaboratively as part of a multidisciplinary outreach team.
  • Conduct follow-up visits to support continued engagement and connection to care.

The Quick Response Team (QRT) model has proven effective in connecting individuals with substance use disorders to treatment and support services, ultimately reducing mortality. QRTs are typically made up of a small, multidisciplinary team (e.g., peer recovery coaches, harm reduction providers, social workers, nurses, behavioral health specialists, community health workers, EMTs/paramedics, and other clinical or non-clinical professionals).

Key Responsibilities:

Peer Recovery Coach—Emergency Department Integration Program

The Peer Recovery Coach (PRC) in the Emergency Department (ED) Integration Program at Corewell Royal Oak and Corewell Troy provides compassionate, peer-based recovery support to individuals presenting to the Emergency Department with substance use–related concerns.

Using lived experience in recovery, PRCs engage patients during moments of crisis, provide emotional support, and connect individuals to treatment, recovery, and community-based services. This role bridges hospital care and long-term recovery, helping patients build stability, hope, and a path forward beyond the Emergency Department.

Applicants must be in long-term recovery from a substance use disorder and able to provide appropriate documentation.

Key Responsibilities:

  • Provide peer recovery support to patients presenting to the Emergency Department due to overdose, substance-related crises, or by request for recovery assistance
  • Respond promptly to ED consults (in person or via telehealth) once patients are medically cleared
  • Build rapport using motivational interviewing, trauma-informed approaches, and recovery-oriented engagement
  • Collaborate with ED staff, social workers, and behavioral health teams to develop individualized recovery plans and facilitate warm handoffs to treatment and community services
  • Coordinate referrals to medication for opioid use disorder (MOUD), detoxification, outpatient treatment, housing, transportation, and community-based PRC services
  • Distribute naloxone kits and provide brief overdose prevention education
  • Document all encounters and follow-up activities in the Electronic Health Record (EHR) and PRC tracking systems accurately and in a timely manner
  • Conduct follow-up outreach within 24–72 hours and at scheduled intervals to support continued engagement in recovery
  • Collaborate with community partners to ensure continuity of care and track referral outcomes
  • Collect and report program metrics including patient engagement, naloxone distribution, and follow-up outcomes
  • Participate in supervision, case reviews, and quality improvement meetings
  • Maintain confidentiality and comply with HIPAA and 42 CFR Part 2 requirements
  • Represent Face Addiction Now and the Navigate PRC program professionally at all times

Combining this experience with training and supervision, coaches will guide families in responding to their loved one in a way that is healthy and supports family recovery. Verified completion of CCAR certification is required. Highly-qualified candidates may be provided CCAR training upon hire. The Family Recovery Coach will be connected to and work with the families of loved ones suffering from substance use disorder. They will provide compassion, support, and resources to help families cope with specific issues, including enabling, codependency, setting healthy boundaries, and other family dynamics.

The Call Center Hopeline is a service provided by Face Addiction Now (FAN) as a part of our Hope Not Handcuffs program. It is designed to connect individuals to treatment options that lead to long-term recovery and reduce the number of overdose fatalities.

FAN is seeking a marketing intern to assist with the organization’s growing communications efforts. We’re looking for someone with creative energy who can contribute new ideas and develop content that highlights our services, program offerings, and upcoming events.

The intern will assist the marketing team on a variety of projects including branding, content creation, social media, and marketing materials. This role also offers the opportunity to contribute to broader campaigns that align with FAN’s organizational goals.

FAN is offering an opportunity to provide support to its overdose education and naloxone distribution (OEND) program, a longstanding public health strategy in response to the opioid crisis. Responsibilities of this intern will include but not be limited to preparing materials, promoting public awareness of opioid risk and overdose prevention, and improving data collection to evaluate program outcomes.

FAN is seeking an intern to assist in the management and growth of our resource hub, focusing on connecting individuals impacted by substance use disorder (SUD) to social resources. This is an opportunity to learn about the different resources available throughout the state of Michigan.

The resource hub is a central collection of tools and referrals used by call center staff, case managers, and other direct service team members to assist individuals across the region. FAN is seeking someone who is committed to helping others and interested in improving service connections and outreach.
