Case Manager—Macomb County/Oakland County

The ReDirect addiction-related diversion program is a street-based, long-term, and unconventional approach to case management. ReDirect case work is non-coercive, non-time delimited, street-based, and built on multiple pathways to recovery, including harm reduction, and steeped in Stages of Change theory and motivational interviewing practices.

This position will manage a caseload of approximately 15-25 participants and will work in conjunction with local police departments and FAN’s Comeback Quick Response Team (QRT). Our team works to identify individuals impacted by substance use disorder (SUD) and provide a continuum of care to ensure participants get the treatment, support, and ongoing connection they need for their recovery journey.

Key Responsibilities:

  • Meeting participants where they're at, both throughout the community and in their recovery process (in-home case management).
  • Transporting participants.
  • Promoting the development of a comprehensive service delivery system by identifying participant needs and gaps in services to meet those needs better.
  • Collaborating with FAN’S Quick Response Team while carrying out duties of case manager.
  • Providing assessments for participants within the program and meeting with participants shortly after identified (within 24 hours).
  • Conducting initial assessments of the client’s needs, desires, strengths, and barriers.
  • Together with the client, preparing the necessary action steps for the client and case manager to achieve established goals.
  • Setting up regular follow-up meetings with the client, based on the client’s needs for intensity – but no less frequently than once per week.
  • Assisting participant in accessing services, resources, and supportive benefits, as needed.
  • Intensive case management, harm reduction strategies, identifying proper treatment, and following-up across 12 months with each participant.
  • Following up, recommending exit, or reevaluating plan, as needed.
  • Attending and participating in case review meetings with Case Management Supervisor at FAN.
  • Tracking and developing appropriate data measures for program evaluation on electronic management system.
  • Supporting and effectively communicating with team members, including employees and contractors.
  • Staying knowledgeable about all FAN programs and national trends.
  • epresenting FAN in a professional manner while performing duties.
  • Other duties as assigned.

Qualifications, Skills & Experience:

  • Bachelor’s degree in the human services or related field, with a proven track record of successfully working with diverse populations. Please note the FAN will consider alternative, comparable experience to a degree.
  • BSW or equivalent preferred.
  • Excellent time management skills, with the ability to work independently and responsibly.
  • Excellent verbal and written communication skills.
  • Comfortable speaking with and in front of law enforcement.
  • Willingness to adhere to the NASW Code of Ethics.
  • Exhibit a commitment to diversity and issues related to poverty.
  • Be culturally competent, including working with those who have serious mental illnesses, who are suffering from chemical dependency, or who have varying gender identifications or sexual preferences.
  • Ability to multitask.
  • Computer skills, with proficiency in Microsoft Office, including Word, Excel, Exchange, and use of the Internet in locating resources.
  • Minimum of 3 years experience working with persons who suffer from substance use disorders (SUD) and mental illness; or 1 academic year of graduate practicum/internship experience working with persons with SUD and mental illness.
  • Ability to implement community program and initiatives while working collaboratively with multiple community organizations and department stakeholders.
  • Ability to work non-traditional hours and be available for critical incident call-ins.
  • Excellent oral and written communication skills, customer service skills, and organizational skills
  • Must be able to complete extensive documentation.
  • Minimum of one year experience with clinical diagnostic assessment and evaluation of adults and children experiencing behavioral health crises, crisis intervention, grief counseling, and determining hospital eligibility is highly preferred.
  • Knowledge of Google products, including Drive, Gmail, and Calendar.
  • Valid Michigan’s driver’s license and proof of insurance.
  • Knowledge of opiate use prevention.

Quick Response Team Program Coordinator

The Quick Response Team (QRT) program is a community-based, proactive outreach initiative that utilizes a multidisciplinary team to provide compassionate, person-centered support to individuals with substance use disorder (SUD) and their families. Operating within deflection and diversion models, the program engages individuals who have recently interacted with law enforcement or first responders due to substance use, connecting them to services and support utilizing a trauma-informed, person-centered approach. In partnership with multiple police agencies and QRT-trained law enforcement personnel, the team conducts home visits to individuals and families affected by SUD.

The QRT Program Coordinator is responsible for managing and coordinating the Face Addiction Now law enforcement–led Quick Response Team program. This role oversees daily operations, staff support, interagency collaboration, and program development to ensure effective service delivery, strong community partnerships, and expanded access to care.

Key Responsibilities:

● Coordinate and oversee daily operations of the COMEBACK Quick Response Team (QRT) program, including troubleshooting issues and resolving problems as they arise
● Serve as the primary liaison between Face Addiction Now, participating law enforcement agencies, and community partners
● Comfortable speaking with and in front of law enforcement
● Collaborate with Law Enforcement Coordinators to ensure effective communication, scheduling, and response efforts
● Coordinate multidisciplinary team activities, including timely follow-up and home visits to individuals and families affected by SUD
● Ensure delivery of person-centered, trauma-informed, and recovery-oriented services
● Track and monitor program data, outcomes, and performance metrics; prepare reports as needed
● Identify gaps in services and work to expand resources, partnerships, and program reach
● Provide guidance, training support, and ongoing communication to QRT-trained law enforcement personnel and team members
● Maintain compliance with organizational policies, confidentiality standards, and applicable regulations
● Assist with program development, strategic planning, and continuous quality improvement initiatives
● Support community outreach efforts to increase awareness of FAN services and reduce stigma
● Manage program logistics, including supply coordination and resource distribution
● Remain knowledgeable regarding SUD best practice approaches and national trends
● Facilitates QRT training for staff and community partners
● Support and effectively communicate with team members including FAN leadership, law enforcement partners, additional stakeholders, FAN staff, and contractors
● Implement administrative systems, procedures, and policies as directed
● Mentor and train new QRT staff, contractors, and interns
● Support QRTs in the field as needed and when available.
● Remain knowledgeable about all FAN programs and national trends (attend training as required)
● Represent FAN/Comeback QRT in a professional manner while performing duties
● Assist with other duties as assigned

Qualifications, Skills & Experience:

● Bachelor’s degree in social work, public health, criminal justice, psychology, or a related field with a proven successful track record of working with diverse populations. Please note that FAN will consider alternative, comparable experience
● Experience collaborating with law enforcement, first responders, or criminal justice systems strongly preferred
● Experience as a person in recovery from substance use disorder or person with family SUD experience strongly preferred
● Minimum of 2–4 years of experience in substance use disorder (SUD), behavioral health, community outreach, or a related field; or one academic year of graduate practicum/internship experience working with persons with SUD and other co-occurring disorders
● Ability to implement community program and initiatives while working collaboratively with multiple community organizations and stakeholders
● Excellent verbal and written communication skills, including the ability to collaborate with diverse stakeholders such as law enforcement, healthcare providers, and community partners
● Ability to problem-solve, think critically, and make sound decisions in dynamic or high-pressure situations
● Strong organizational and time-management skills with the ability to manage multiple priorities and respond to emerging needs

Hope Line Call Center Follow-Up Specialist

The Hope Line Call Center Follow-Up Specialist is responsible for supporting individuals and families impacted by substance use. This role focuses on reconnecting with participants, conducting follow-up calls, ensuring accurate data, and helping individuals access treatment, recovery support, and community resources.

Key Responsibilities:

  • Conduct follow-up calls to support engagement in FAN programs
  • Reconnect with inactive participants and assist in re-engagement
  • Close out outdated cases while ensuring data accuracy
  • Support individuals with open cases to improve outcomes
  • Connect participants to treatment providers, community resources, and FAN peer and family coaches
  • Collaborate with staff members and case managers across programs
  • Accurately document all interactions
  • Support and effectively communicate with all FAN staff members
  • Participate in team collaboration and ongoing communication across programs
  • Adjust hours based on community needs, including occasional evenings and one weekend per month

Peer Recovery Coach—Washtenaw County/Wayne County

The Peer Recovery Coach provides support and mentorship to individuals seeking recovery from substance use disorder, using their own lived experience to build trust and foster a supportive, professional relationship. This role involves empowering participants to create and maintain their own recovery plan, connecting them with community resources, and providing emotional encouragement to overcome barriers to recovery.

Key Responsibilities:

  • Provide support to individuals seeking recovery from substance use disorder, while establishing a professional relationship based on trust and lived experience.
  • Meet individually with participants, ideally in-person, or virtually, by phone, or in various community settings.
  • Share personal stories related to substance use and recovery to inspire and empower participants.
  • Empower individuals to create and maintain their own recovery plan.
  • Provide emotional and influential support while connecting participants with community resources based on their needs, including self-help groups, social service agencies, and life skills classes.
  • Connect individuals with life skills and resources, while acting as a role model and mentor.
  • Provide resources to remove barriers to recovery, while offering hope, motivation, and encouragement.
  • Attend bi-weekly team meetings, monthly FAN forums, and agency events related to coaching.
  • Complete coaching documentation concisely and efficiently.
  • Participate in monthly case-review sessions focusing on participant-centered care.
  • Maintain confidentiality of participants in accordance with state and federal guidelines (HIPAA).
  • Represent the organization in a professional manner in all regards.
  • Maintain knowledge of all FAN programs and national trends in substance abuse prevention and treatment, including harm reduction.
  • Be supportive of all paths to recovery.
  • Set and manage personal and professional boundaries effectively.

FAN is seeking a marketing intern to assist with the organization’s growing communications efforts. We’re looking for someone with creative energy who can contribute new ideas and develop content that highlights our services, program offerings, and upcoming events.

The intern will assist the marketing team on a variety of projects including branding, content creation, social media, and marketing materials. This role also offers the opportunity to contribute to broader campaigns that align with FAN’s organizational goals.

NALOXONE PROGRAM SUPPORT

FAN is offering an opportunity to provide support to its overdose education and naloxone distribution (OEND) program, a longstanding public health strategy in response to the opioid crisis. Responsibilities of this intern will include but not be limited to preparing materials, promoting public awareness of opioid risk and overdose prevention, and improving data collection to evaluate program outcomes.
