Case Manager—Redirect Addiction-Related Diversion Program (Macomb/Oakland County)

The Redirect addiction-related diversion program is a street-based, long-term, and unconventional approach to case management. Redirect case work is non-coercive, non-time delimited, street-based, and built on multiple pathways to recovery, including harm reduction, and steeped in Stages of Change theory and motivational interviewing practices.

This position will manage a caseload of approximately 15-25 participants and will work in conjunction with local police departments and FAN’s Comeback Quick Response Team (QRT). Our team works to identify individuals impacted by substance use disorder (SUD) and provide a continuum of care to ensure participants get the treatment, support, and ongoing connection they need for their recovery journey.

Duties will include:

  • Meeting participants where they're at, both throughout the community and in their recovery process (in-home case management).
  • Transporting participants.
  • Promoting the development of a comprehensive service delivery system by identifying participant needs and gaps in services to meet those needs better.
  • Collaborating with FAN’S Quick Response Team while carrying out duties of case manager.
  • Providing assessments for participants within the program and meeting with participants shortly after identified (within 24 hours).
  • Conducting initial assessments of the client’s needs, desires, strengths, and barriers.
  • Together with the client, preparing the necessary action steps for the client and case manager to achieve established goals.
  • Setting up regular follow-up meetings with the client, based on the client’s needs for intensity – but no less frequently than once per week.
  • Assisting participant in accessing services, resources, and supportive benefits, as needed.
  • Intensive case management, harm reduction strategies, identifying proper treatment, and following-up across 12 months with each participant.
  • Following up, recommending exit, or reevaluating plan, as needed.
  • Attending and participating in case review meetings with Case Management Supervisor at FAN.
  • Tracking and developing appropriate data measures for program evaluation on electronic management system.
  • Supporting and effectively communicating with team members, including employees and contractors.
  • Staying knowledgeable about all FAN programs and national trends.
  • Representing FAN in a professional manner while performing duties.
  • Other duties as assigned.

Qualifications:

  • Bachelor’s degree in the human services or related field, with a proven track record of successfully working with diverse populations. Please note the FAN will consider alternative, comparable experience to a degree.
  • BSW or equivalent preferred.
  • Excellent time management skills, with the ability to work independently and responsibly.
  • Excellent verbal and written communication skills.
  • Comfortable speaking with and in front of law enforcement.
  • Willingness to adhere to the NASW Code of Ethics.
  • Exhibit a commitment to diversity and issues related to poverty.
  • Be culturally competent, including working with those who have serious mental illnesses, who are suffering from chemical dependency, or who have varying gender identifications or sexual preferences.
  • Ability to multitask.
  • Computer skills, with proficiency in Microsoft Office, including Word, Excel, Exchange, and use of the Internet in locating resources.
  • Minimum of 3 years experience working with persons who suffer from substance use disorders (SUD) and mental illness; or 1 academic year of graduate practicum/internship experience working with persons with SUD and mental illness.
  • Ability to implement community program and initiatives while working collaboratively with multiple community organizations and department stakeholders.
  • Ability to work non-traditional hours and be available for critical incident call-ins.
  • Excellent oral and written communication skills, customer service skills, and organizational skills
  • Must be able to complete extensive documentation.
  • Minimum of one year experience with clinical diagnostic assessment and evaluation of adults and children experiencing behavioral health crises, crisis intervention, grief counseling, and determining hospital eligibility is highly preferred.
  • Knowledge of Google products, including Drive, Gmail, and Calendar.
  • Valid Michigan’s driver’s license and proof of insurance.
  • Knowledge of opiate use prevention.
  • Articulate and approachable.

  • Ability to lift to 25 lbs., and sit, stand, bend, climb, lift, carry, and/or reach frequently and for extended periods of time.

Care Coordinator/Community Health Worker—Mobile Clinic & Hep C Services

(Two positions available.)

\The Care Coordinator/Community Health Worker plays a vital role in supporting hepatitis C (HCV) outreach, linkage to care, treatment coordination, and follow-up services through a mobile clinic setting. This position works closely with individuals disproportionately impacted by HCV, HIV, sexually transmitted infections (STIs), substance use disorders, homelessness, and co-occurring mental health conditions.

This role is responsible for conducting community outreach, building trusting relationships with individuals in underserved communities, supporting engagement in HCV treatment, and helping clients navigate health and human service systems. The Care Coordinator collaborates closely with an interdisciplinary team to provide patient-centered care and improve health outcomes among vulnerable populations.

Key Responsibilities:

Outreach & Community Engagement

  • Conduct targeted outreach to individuals at high risk for HCV, HIV, and STIs, including individuals impacted by homelessness, substance use disorders, or co-occurring mental health conditions
  • Engage in street outreach and mobile clinic activities to build supportive, nonjudgmental relationships with clients
  • Build and maintain rapport with individuals needing linkage to medical care, treatment, and supportive services
  • Provide education regarding HCV prevention, treatment adherence, and available community resources
  • Provide rapid HCV, HIV, and STI testing for clients
  • Assist with mobile clinic operations, including restocking supplies, maintaining organization and cleanliness of the unit, and preparing materials needed for outreach activities
  • Establish and maintain formal and informal partnerships with organizations that have similar or allied goals
  • Represent the organization professionally in community settings and maintain knowledge of organizational programs

Care Coordination & Client Support

  • Coordinate linkage to care for individuals who test positive for HCV, HIV, or STIs
  • Support individuals throughout HCV treatment by assisting with follow-up appointments, medication adherence, and ongoing engagement in care
  • Deliver medications to clients as needed
  • Assess client needs and provide referrals to appropriate health and social service resources
  • Collaborate with nurses, peer recovery coaches, case managers, and program leadership to deliver high-quality, person-centered care
  • Document client interactions and services in a timely and accurate manner and assist with program reporting requirements
  • Maintain confidentiality in accordance with HIPAA, 42 CFR Part 2, and organizational policies
  • Support all pathways to recovery using a compassionate, trauma-informed, and person-centered approach
  • Attend required meetings and trainings and perform other duties as assigned

Qualifications, Skills & Experience:

  • Community Health Worker certification required or willingness to obtain certification
  • Must have a valid driver’s license, be insurable under the organization’s policy, and have access to reliable transportation. Use of a personal vehicle may be required at times as part of job responsibilities
  • Experience working with individuals affected by substance use disorders, homelessness, mental health conditions, or other vulnerable populations strongly preferred
  • Comfortable working in both mobile outreach and clinic-based environments
  • Strong interpersonal, communication, and relationship-building skills
  • Ability to maintain professional boundaries and work effectively within a team-based environment
  • Knowledge of Microsoft Office and Google Workspace applications
  • Valid driver’s license, reliable transportation, and ability to be insured under the organization’s policy
  • Ability to pass a background check, sex offender registry check, and drug screening within 48 hours of offer
  • Ability to frequently sit, stand, bend, lift, carry, and reach throughout the workday, including lifting up to 25 pounds

The Call Center Hopeline is a service provided by Face Addiction Now (FAN) as a part of our Hope Not Handcuffs program. It is designed to connect individuals to treatment options that lead to long-term recovery and reduce the number of overdose fatalities. 

MARKETING

FAN is seeking a marketing intern to assist with the organization’s growing communications efforts. We’re looking for someone with creative energy who can contribute new ideas and develop content that highlights our services, program offerings, and upcoming events.

The intern will assist the marketing team on a variety of projects including branding, content creation, social media, and marketing materials. This role also offers the opportunity to contribute to broader campaigns that align with FAN’s organizational goals.

NALOXONE PROGRAM SUPPORT

FAN is offering an opportunity to provide support to its overdose education and naloxone distribution (OEND) program, a longstanding public health strategy in response to the opioid crisis. Responsibilities of this intern will include but not be limited to preparing materials, promoting public awareness of opioid risk and overdose prevention, and improving data collection to evaluate program outcomes.

The resource hub is a central collection of tools and referrals used by call center staff, case managers, and other direct service team members to assist individuals across the region. FAN is seeking someone who is committed to helping others and interested in improving service connections and outreach.
