Peer Recovery Coach–Detroit Quick Response Team

The Peer Recovery Coach (PRC) is a key part of FAN’s Detroit Quick Response Team, providing immediate and ongoing support to individuals impacted by substance use. This role is rooted in lived experience, compassion, and the belief that recovery should be met with dignity and without judgment.

This position combines the principles of peer support and community outreach. The PRC works side-by-side with an Outreach Specialist, responding to overdose-related calls, community referrals, and follow-up needs across Detroit.

FAN’s QRT model is based on proven strategies used across the country to reduce overdose deaths and promote recovery by meeting people where they are—on the street, in their homes, or wherever help is needed most.

Responsibilities will include:

  • Engage individuals in a trauma-informed, person-centered way during field outreach, including overdose follow-up visits.
  • Offer emotional support, encouragement, and a connection to care that honors all paths to recovery.
  • Link individuals to treatment, harm reduction services, peer support, and basic needs resources.
  • Share your personal recovery experience to foster trust and demonstrate the reality of recovery.
  • Help remove barriers to recovery (e.g., transportation, documentation, stigma).
  • Document interactions and outcomes clearly in the case tracking system.
  • Maintain up-to-date knowledge of FAN services, community resources, and trends in recovery and harm reduction.

Requirements:

  • Minimum of 3 continuous years in recovery.
  • High school diploma or GED.
  • Valid driver’s license and insured, reliable transportation.
  • Must pass a background check, sex offender registry check, and a drug screen within 48 hours of hire.
  • Completion of CCAR Recovery Coach Academy (or willingness to complete the 5-day course before starting).
  • Completion of 16-hour CCAR Ethics within 4 months of hire.
  • Strong understanding of trauma-informed care and harm reduction.
  • Ability to maintain boundaries and model professionalism.
  • Experience with community-based outreach or street outreach preferred.
  • Tech-savvy: Proficiency with Google Workspace (Drive, Gmail, Sheets, Calendar) and Microsoft Office Suite.

Community Response Navigator–Detroit Quick Response Team

The Community Response Navigator for the Detroit Quick Response Team (QRT) plays a vital role in supporting daily operations and ensuring timely response to overdose incidents and public safety referrals. Acting as the central point of coordination, this position organizes outreach activities, manages real-time data, and supports the team both in the office and in the field when needed.

Most shifts will be scheduled from 6:00 AM to 2:00 PM, with rotating weekend coverage required. Flexibility in hours is essential, based on the needs of the team and the community.

Responsibilities will include:

Overdose & Outreach Coordination

  • Monitor and enter overdose data into the internal tracking system.
  • Contact area hospitals to confirm patient status for follow-up outreach.
  • Prepare and prioritize QRT visit forms based on urgency (non-transports, recent overdoses, follow-ups).
  • Re-route teams in real time based on incoming overdose alerts or hospital discharges.
  • Track and update shift activity reports using internal spreadsheets.

Operational Support

  • Participate in outreach efforts when additional support is needed.
  • Assist with hospital responses and overdose follow-ups in the field.
  • Support the day-to-day functioning of the Detroit QRT office, including organizing materials and keeping team tools updated and accessible.
  • Create and assemble harm reduction/outreach kits.
  • Help coordinate volunteers and assist with onboarding.

Requirements:

  • Strong organizational and time management skills; ability to prioritize in fast-paced situations.
  • Effective communicator with the ability to coordinate a mobile team in real-time.
  • High attention to detail and accuracy in data entry and documentation.
  • Comfortable using spreadsheets, email, and data tracking tools (Google Workspace preferred).
  • Ability to work independently and as part of a collaborative team.
  • Compassionate and nonjudgmental approach to working with individuals impacted by substance use.
  • Flexibility in schedule, including early mornings and rotating weekends.
  • Commitment to confidentiality and ethical practices.

Preferred Experience:

  • Experience in harm reduction, behavioral health, peer support, or emergency response.
  • Familiarity with Detroit communities and public safety or healthcare systems.
  • Lived experience or a strong understanding of substance use disorder and recovery is a plus.

Case Manager–Detroit Quick Response Team

The Case Manager for the Detroit Quick Response Team (QRT) provides intensive, long-term support to individuals impacted by substance use disorder, with a specific focus on co-occurring mental health challenges and barriers related to housing, income, or employment. This role is responsible for outreach, case management, transportation, and systems coordination aimed at improving participant stability and overall quality of life.

The Case Manager also plays a key role in managing follow-up efforts, tracking outcomes, and building strong relationships within Detroit’s criminal justice and housing systems to strengthen service connections.

Responsibilities will include:

Outreach & Immediate Engagement

  • Participate in field-based outreach alongside DQRT team members following non-fatal overdoses or public safety referrals.
  • Conduct home and community visits to offer support, assess needs, and build trust with participants.
  • Provide harm reduction education and distribute resources such as naloxone, hygiene supplies, and referral materials.

Case Management & Coordination of Care

  • Deliver person-centered, intensive case management for participants with complex needs, supporting long-term recovery and stability.
  • Develop individualized support plans that address substance use, mental health, and barriers related to housing, income, or employment.
  • Coordinate services across treatment providers, housing agencies, workforce development programs, and the criminal justice system.
  • Provide transportation to treatment, housing, legal, and support service appointments to reduce barriers to engagement.
  • Maintain regular contact with participants and offer consistent follow-up for up to 12 months.

Outcome Management & Quality of Life Focus

  • Track participant engagement, service utilization, and progress using FAN’s case management systems.
  • Monitor and document improvements in participants’ quality of life, including health, safety, housing, and purpose.
  • Collaborate with leadership to analyze trends and contribute to continuous program improvement.

System Collaboration & Advocacy

  • Build and sustain strong working relationships with courts, probation departments, shelters, behavioral health providers, and reentry programs.
  • Serve as a liaison between participants and systems partners, advocating for timely, equitable access to needed services.
  • Participate in local coalitions, multidisciplinary teams, and community initiatives aimed at reducing barriers and improving coordination.

Requirements:

  • Bachelor’s degree in Social Work, Human Services, Criminal Justice, or related field required — or equivalent professional/lived experience.
  • Minimum of 2 years of experience in case management, outreach, or behavioral health services.
  • Working knowledge of substance use disorder, harm reduction principles, mental health supports, and Detroit’s housing and justice system resources.
  • Ability to build rapport with diverse populations and respond effectively in high-stress or crisis situations.
  • Must have a valid driver’s license, reliable vehicle, and current auto insurance (mileage reimbursement provided).

Preferred Experience:

  • Experience supporting justice-involved individuals or those experiencing homelessness.
  • Familiarity with community-based crisis response or deflection/diversion programming.
  • Lived experience in recovery or system involvement is valued and respected.

Family Recovery Services Supervisor

Face Addiction Now (FAN) is seeking a passionate and experienced Family Recovery Services Supervisor to lead and grow our statewide family support programs. This full-time, grant-funded position is based in Clinton Township, MI and plays a critical leadership role in providing oversight, supervision, and strategic direction for FAN’s Family Recovery Coaching services and Stronger Together support groups.

The ideal candidate has lived experience as a family member of someone affected by substance use disorder (SUD), along with a background in supervision, social services, or peer recovery. This position is responsible for supervising Family Recovery Coaches, launching and supporting peer-led family groups, and promoting FAN’s mission to help families heal and thrive. Position Overview:

The Family Recovery Services Supervisor is responsible for overseeing all aspects of FAN’s Family Recovery Coaching services, including supervision and support of Family Recovery Coaches (FRCs), development and facilitation of Stronger Together support groups, and the promotion and strategic expansion of FAN’s family services. This leadership role ensures high-quality support to families affected by substance use disorder, alignment with FAN’s mission, and intentional growth of services across the state.

Primary duties and responsibilities will include:

  • Provide direct supervision to Family Recovery Coaches, including routine case reviews, support, and documentation audits.
  • Ensure adherence to FAN standards, grant requirements, and ethical guidelines.
  • Respond promptly to concerns or feedback about coach performance or program experience.
  • Lead onboarding and ongoing training for new and existing FRCs, including FAN procedures, CCAR model, and use of the data platform.
  • Deliver individualized support to FRCs to enhance skill-building and documentation accuracy.
  • Assign caseloads and oversee coach-to-family matching.
  • Monitor data for accuracy, completion, and program compliance.
  • Review and submit timesheets and payroll.
  • Compile monthly reports, outcome metrics, and narratives for FAN leadership and funders.
  • Oversee the implementation and quality of Stronger Together groups across locations.
  • Develop and launch new groups based on community needs, including coordination of logistics, facilitators, and promotion.
  • Support facilitators through check-ins, training refreshers, and resource sharing.
  • upport strategic planning efforts to grow and sustain FAN’s family recovery programming.
  • Work with the leadership team to identify gaps in services and opportunities for expansion in underserved communities.
  • Collaborate across FAN programs to ensure family services are integrated into the full continuum of care.
  • Strengthen relationships with community partners, stakeholders, and referring agencies to enhance program reach and engagement.
  • Serve as liaison between Family Recovery Services and other FAN initiatives.

Requirements:

  • Bachelor’s degree preferred in a related field such as Social Work, Psychology, Public Health, Human Services, or Counseling. Equivalent experience in peer support, recovery services, or program supervision may be considered in lieu of a degree.
  • Lived experience as a family member of someone with a substance use disorder, with the ability to share that experience appropriately in a professional setting.
  • Completion of the CCAR Recovery Coach Academy (post-hire).
  • Experience as a Family Recovery Coach or Stronger Together facilitator preferred.
  • Supervisory or leadership experience required.
  • Strong understanding of family recovery, the impact of SUD, and trauma-informed peer support.
  • Excellent organizational, interpersonal, and communication skills.
  • Experience with strategic planning, program promotion, or community engagement is a plus.
  • Proficiency in Microsoft Office, Google Suite, and data management platforms.
  • Ability to work flexible hours, including evenings and weekends as needed.
  • Valid Michigan driver’s license and reliable transportation.
